We are delighted to accept new patients onto our list, as long as you live within our practice area.We try and organise our practice to encourage continuity of care with patients ideally seeing one doctor through a particular problem.
If you wish to register with the Practice, please complete our online registration form. Please note: Your registration is not complete until you have a new patient check with the health care assistant. To complete your registration with the surgery you will need to visit the practice with proof of Identification and one of the following as proof of address:

We do require all potential patients to bring proof of address. It would also help if you could bring your up to date medical card.
At registration we will need the following information:
We require all new patients to complete a confidential questionnaire about their medical history. This will help the Doctors and Nurses at the Lea Surgery to appreciate your health needs.
If you have any significant medical problems eg diabetes it would be advisable to book an appointment with the doctor after registration. If you are on regular medication please bring it with you to your first appointment, so that repeat prescriptions can be organised.
