Registration Procedure

All UK residents have the right within the NHS to be registered with a Doctor.

We are delighted to accept new patients onto our list, as long as you live within our practice area.We try and organise our practice to encourage continuity of care with patients ideally seeing one doctor through a particular problem. 

If you wish to register with the Practice

If you wish to register with the Practice, please complete our online registration formPlease note: Your registration is not complete until you have a new patient check with the health care assistant. To complete your registration with the surgery you will need to visit the practice with proof of Identification and one of the following as proof of address:

  • Utility Bill (within the last 3 months)
  • Bank Statement (within the last 3 months)
  • Tenancy Agreement

Register at the Practice

You can also register by visiting the Lea Surgery during our opening times Monday to Sunday, and speak to our receptionists.

Information we will need: 

We do require all potential patients to bring proof of address. It would also help if you could bring your up to date medical card.
At registration we will need the following information:

  • Full name, including previous names
  • Date and place of birth
  • Last address
  • New address and post code
  • Last name and address of your previous Doctor
  • NHS number if known (not the same as your National Insurance number)
  • Proof of residence eg utility bill
  • Red book/immunisation record for children aged 5 and under
    If you are a recent arrival to the UK we may need to see your passport and the visa.
  • If you are an asylum seeker we will need to see official documentation inc IND number.

We require all new patients to complete a confidential questionnaire about their medical history. This will help the Doctors and Nurses at the Lea Surgery to appreciate your health needs.

If you have any significant medical problems eg diabetes it would be advisable to book an appointment with the doctor after registration. If you are on regular medication please bring it with you to your first appointment, so that repeat prescriptions can be organised.
